Beginning January 1, 2021, your pharmacy benefit plan coverage for blood glucose test strips and meters may be changing. ProAct’s preferred blood glucose testing supplies will be transitioning from Accu-Chek® and One Touch® brand products to Contour®Next brand products made by Ascensia Diabetes Care.
To help support members in this transition we encourage you to Enroll in our Diabetes Management Program. Once enrolled, log in, click on the My Resources tab. Then choose Diabetes Management for helpful links and savings opportunities on Contour®Next meter and test strips.

Making this change will require a prescription from your prescriber for Contour®Next test strips to be filled on or after January 1, 2021. Contour®Next test strips are:

• Compatible with all Contour®Next branded meters
Once enrolled in the Contour Choice Program, eligible customers can save up to $105 per month on test strip copays.†
